Consent to personal data processing

In accordance with Article 9 of Federal Law No. 152-FZ dated 27.07.2006 "On Personal Data", in order to fulfil the obligations arising from accepting the public offer for a donation agreement, I give my consent to the processing of personal data to the State Budgetary Cultural Institution of the City of Moscow "M.A. Bulgakov Museum" (OGRN 1077763509020, INN 7710699594, registered address: 123001, Moscow, Bolshaya Sadovaya str., 10, floor 4, premises 50, room 1) (hereinafter — the Operator).

The list of my personal data for whose processing I give consent: full name, bank details, email address, phone number.

I authorise the Operator to carry out automated and non-automated processing of my personal data: collection, storage, use, de-identification, blocking, and destruction of personal data.

This consent is valid for 2 (two) years from the date it is given and may be withdrawn at any time by written application.
